Registration for each season begins May 1st

Once your roster is complete, it takes a week to 10 days to get the player and coaches cards back. All of the registration packets must go through our club registrar Kristen Barrett, then submitted to the League Registrar, Kim Waltz.

EACH PLAYER

 

 

 

  • USYSA form (2 part) must be used and filled out completely including all signatures. Remember player must print and sign name as shown on birth certificate.
  • All new player registration forms must be accompanied by a photocopy of a stated stamped birth certificate.
  • Player picture cut to 1” by 1” square (paperclip to form with name on back.
  • No staples.

 

 

 

EACH COACH

 

 

 

 

  • Coaches registration form filled out completely.
  • Photocopy of coaching license.
  • Coach picture cut to 1” by 1” square with name on back & paperclip to license.
  • Adult add form for anyone requiring a coach pass who does not appear on the team roster. All assistant coaches must be named on form.
  • Employment/Volunteer Disclosure Statement. All coaches and assistant coaches are required for insurance purposes to fill out this form completely.

 

 

 

 

 

One check for the entire team’s registration is to be submitted with complete packet.

 

 

ALL PACKETS ARE DUE BACK TO CLUB REGISTRAR AT THE NEXT CLUB MEETING AND OR NO LATER THAN MAY 15TH. ALL PACKETS MUST BE COMPLETE TO TURN IN. RETURN ALL FORMS. PLAYER CARDS AND COACHES PASSES WILL BE DISTRIBUTED WHEN RECEIVED. YELLOW REGISTRATION FORMS WILL BE PROVIDED PRIOR TO ISSUE OF CARDS.
